If Uk leaves with NO Deal :

Must have microchip

Must Have Rabies Vaccination

Must have Rabies blood test at least 30 days after vaccination

If passes blood test, you must wait 3 months before you can travel with your pet.

Passport will not be valid. Therefore you must visit an official veterinarian (Ani-Medics can do this) and they will issue a single use Animal Health Certificate which will be valid for travel to the EU for up to 10 days post issue.
